We are currently accepting new patients.
How to Register
We can only accept new patients who live in our practice area. (We do not accept 'Out-of-Area' registrations) The Practice serves the population of Old Harlow and neighbouring villages, click on this link to see whether you are in our catchment area.
Instructions on what to do with the registration form
NOTE: You will not be able to book appointments until you have been fully registered. i.e. signed the pre-registration form at the surgery, provided proof of identity and address and completed the additional forms required by the Practice. The Registration process normally takes 48 hours from receipt of the completed forms and surgery checks.
If you want to join the practice please complete a registration form which can be picked up from reception or downloaded from the above “Registering Online” tab. Please bring the completed form along to reception with your medical card if you have it, proof of address i.e. utility bill or council tax statement and also photographic ID i.e. passport, driving licence.
All newly registered patients need to complete a health questionnaire and have a health check with the practice nurse.
The information on the medical card will speed up the process of getting your records from your previous practice. Once you have been accepted as a patient your medical records will be transferred to us and a new medical card will be sent to your home address. You do not need to inform your existing practice that you are moving, although it may be courteous to do so.
The Practice boundary for new patients is defined as follows:
